MFuge Participant Form – MUST complete if you are going on the trip

Our Mfuge trip to Philadelphia is just 4 weeks away on July 19-26, 2009.  Please complete and have notarized two copies of the participant form.  All participants need to turn in a copy of the insurance card as well.  Please have your Mfuge balance paid by this Sunday, June 28th.  Call the church office to confirm your balance.
